Registration

| Registration information can be obtained from our front office anytime between 8:15 & 3:45pm. |
| IN ORDER TO COMPLETE YOUR CHILD'S REGISTRATION, YOU MUST HAVE: |
| 1) Your Child's Birth Certificate |
| 2) A Current Florida Driver's License (with your CORRECT address). |
| 3) Physical (completed within the past year) |
| 4) Proof of Immunizations (on blue Florida form) |
| 5) Proof of Residency (lease, warranty deed, Homestead Exemption card, etc.) |
